- I give permission for my child to attend and participate in all activities within the Sydney Futsal program selected.
- By enrolling at Sydney Futsal Academy, you have agreed to join and pay the total cost of the program, which is non-refundable. If you decide you do not wish to continue with the program, you must contact Sydney Futsal by emailing Sydneyfutsalacademy@gmail.com. to cancel your booking.
- Your child’s place in Sydney Futsal Academy program is not secured until fees are paid in full and you receive a confirmation email from us.
- All fees/charges paid are non-refundable.
- All reasonable care and caution are taken to ensure the safety and wellbeing of our players. However, Sydney Futsal Academy cannot accept liability for any injury that may occur whilst attending any of our programs.
- Parents/guardians and children participate at their own risk and therefore indemnifies Sydney Futsal Academy and its staff against all actions, suits, proceedings, demands, costs and expenses which may be taken or made against Sydney Futsal Academy which may arise from injury, illness or mishap.
- Sydney Futsal Academy will not be held responsible for any loss or damage to any personal belongings while attending one of our programs.
- Any medical conditions that a participant may have must be fully disclosed to Sydney Futsal Academy in the registration forms.
- Parents, guardians and children in their care are enrolled at their own risk and you as the parent/guardian agree to indemnify Sydney Futsal Academy and its staff from and against any and all claims, demands, actions, injury and/or damages that may arise from you or your child’s participation in any Sydney Futsal Academy programs or sessions.
- All parents and guardians give permission for their child to receive medical/ambulance attention in case of emergency and agree to pay any associated costs.
- Sydney Futsal Academy reserves the right to limit the number of participants involved in its programs and/or cancel any of its programs at any time but will endeavour to give at least 48 hours’ notice prior to the event
- Sydney Futsal Academy may occasionally take photos or video footage for promotional purposes, please advise staff if you do not endorse your child’s image use for promotional purposes.
- In completing your child’s registration and enrolling in one of our programs, you acknowledge and agree to these Terms and Conditions.
